Dear children and parents,
If you wish to begin your journey into the magical world of Wishes, all you need to do is complete the request form below.
Step 1:
Select the language in which you would like the form to appear, in the field “Please select your Language.”
Then, fill in the child’s information exactly as it appears on official documents.
Once all required fields are completed, click the blue “Next” button located at the bottom right of the screen.
Step 2:
Fill in the details of the legal guardian, who will be the primary point of contact.
These details must also match the information shown on official documents.
After completing all required fields, click the blue “Next” button at the bottom right of the screen.
Step 3:
In the field “Hospital of Treatment,” select from the list the hospital corresponding to the child for whom you are completing the form.
Next, upload the child’s medical report or KE.P.A. document in the field “Attach Medical Report or KE.P.A.”
Don’t forget to fill in the Name and Phone Number of the Attending Physician, so we can contact them directly.
To finalize the process, click the blue “Submit” button at the bottom right of the screen.
